Mine has 'EPSON' repeated across the back. 
I've tweaked the 'EX' to form an arrow.

Now available for download, this file will print the card at approx 85x55mm (credit card size) at 300 dpi.
Simply download, print on heavy stock or photo paper and then trim around the edge. The notches can be tricky. 
Enjoy!
